dude i'm about to passed out, so friggin exhausting today. so i'm in the Medical ICU following the pulm and Critical care team. today, brand new team, no fellow, no regular CCU pharmacist, and 3 new admits. i didn't even eat lunch cuz i was watching the team placing an IJ and A line in a patient, which took forever. and vanco PK dosing up the yin yang!

i'm on amb care and it fuckin sucks ass. O-M-G its so boring and mundane. i'm rotating around all the free clinics in SD, and the 'pharmacy' usually has about one drug for each disease state.
i have 'the mobile health clinic' every thursday, and they put the meds in one of those fucking tool storage wheely thingamajiggy. we hand write the label for every drug. can you believe this shit? oh and the student run clinic is a fuckin dump. and it smells like armpits! yuk!
tell me more about orthosurg. i heard they do speed surgery (kinda like speed dating, except they cut and sew ppl up in 20 secs)

OMG verena. sorry to hear you are not liking gen med! for me, i have a love/hate relationship with it. i've been such a cry baby lately. the first time i cried bc i felt bad bc 1 of my viet patients asked for my # so that she can call me when she has Qs and I told her i can't. and then i cried like a baby yest bc one of my pts died and he reminded me of my grandpa. and then today i cried bc one of my pts who was doing relatively well a couple days ago turned the corner and he might be d/c to comfort care. he could barely hold up a pen to sign the consent form for his paracentesis.
anyway, i feel so DUMB. its not even funny. today, one of the pharmacists was quizzing me on heparin (its nice if they tell you to review it first!!! but they never do) and the px vs tx doses. and i was like?????? i really HATE saying "I don't know, but I will look it up and get back to you." ARGGGH. Why am I so clueless????
I have to page the team too to see when we're rounding. and our rounds vary. i'm not even in charge of anything (admits, discharges etc). we just come early, pre-round by ourselves, and then round. afterwards, i go over notes w/ the res and enter them into our database. then i usu do vanco level or i have to present something to the res/preceptor.
hahaha i feel the same, every time someone has an abx, i always get excited too. pathetic i know. but at least ID is the one thing i'm fairly knowledgeable in. like the other day, the team didn't know if zosyn has good coverage vs anaerobes, and i was like, "Well actually....." LMAO
i was dying last week. i feel like i'm in medical school bc i always gotta look up all the diagnostic tests performed. and abbreviations drive me nuts! ercp/mrcp, vats, egd, tips, cvvhd, ards blah blah. english please!
they also make me do journal club, dx state presentation and an inservice. oh and do vanco level. i had to come in yesterday on my day off after working (paid work) to look over the new admits. how is your typical day like? i think we should have a phone date so that you can teach me something :D.
