Subacute care is an intensive level of care one step down from the kind of acute - and expensive - care provided by a hospital. Thanks to subacute facilities, patients with medically complex conditions requiring such specalized services as respiratory therapy, physical rehabilitation, cardiac care, pain management and wound care can now receive such services in a more cost-effective environment. They are fully staffed with a hospital-trained nurses and provide 24 hour medical management. Today's subacute facilities provide the kind of specalized round-the-clock care post-surgical or post-traumatic patients require in order to attain optimum recovery. The ultimate goal of subacute care is to help patients heal, both physically and emptionally, in a therapeutic environment, until they are well enough to be discharged to a lower level of care, or preferably, their homes. Lakeview offers an extensive physical rehabilitation regimen in our expanded facility. We take patients after the acute rehabilitation phase and continue the program, enabling you to achieve greater independence and self-fulfillment. Together, we teach you the self-help skills required to reach your full potential. Our rehabilitation team consists of these talented professionals: Physiatrist, Physical Terapists, Occupational Therapists, Speech-Language Pathologists and Therapy Assistants.
