Jason is not responsible for any injury, hurt feelings, embarrassment or damage of any kind sustained by any person while knowing him, including injury, hurt feelings, embarrassment or damage which might be caused by the negligence of Jason.
DESCRIPTION OF RISKS
In consideration of my acquaintanceship, I acknowledge that I am aware of the possible risks, humiliation and hazards associated with my acquaintanceship with Jason, (including the possible risk of severe or fatal mental injury to myself or others). These risks include but are not limited to the following:
a) the risks associated with becoming the subject thereof in a blog authored by Jason;
b) intoxication and/or alcohol poisoning from the alcohol I consume whether voluntarily or through coercion;
c) the possibility of mental anguish, embarrassment or psyche injury;
INDEMNIFICATION AND RELEASE OF LIABILITY
In return for Jason allowing me to voluntarily participate in acquaintanceship and related activities, I agree:
1. TO ASSUME AND ACCEPT ALL RISKS arising out of, associated with or related to my participation in our acquaintance, even though such risks may have been caused by the negligence or on purpose by Jason;
2. TO BE SOLELY RESPONSIBLE FOR ANY EMBARRASMENT, MENTAL ANGUISH, LOSS OR DAMAGE which I might sustain in my relationship with Jason, even though such mental anguish or injury that may have been caused by negligence or on purpose by Jason;
3. TO HOLD HARMLESS AND INDEMNIFY JASON:
a) from any and all liability for any damage to the personal property of, or personal injury to, any third party resulting from my acquaintanceship with Jason and all related activities; and
b) from any and all claims, demands, actions and costs which might arise out of my acquaintanceship with Jason, even though such claims, demands, actions and costs may have been caused by the negligence of Jason.
By reading this, I consent to having the information in this document collected by Jason. The personal information requested on this form is collected under the perceived authority of Jason. Certain personal information may be made available to federal and state government departments and agencies under appropriate legislative authority.
ACKNOWLEDGEMENT
I acknowledge that I have read this agreement, that I have executed this agreement voluntarily, and that this agreement is to be binding upon myself, my heirs, executors, administrators and representatives, in the event of my death or incapacity.
