The original version of Dippermouth Blues was recorded in 1923 - King Oliver with Louis Armstrong on backup cornet (the first recording of a black jazz band that wasn't just pickup members, actually). The song title is actually a nod to Louis; his embrochoure made him look like a dippermouth bass (he picked up "Satchelmouth", later "Satchmo" for the same reason).

As I remember the story, one of the musicians was supposed to hit a riff on that break, and completely missed it; in a flash of excitement and inspiration, someone else yelled out the reminder. Afterwards, it was just too damn good to take out, and the rest is music history.

Many of the musicians in those days were REALLY paranoid about people stealing and/or copying their music and styles (Freddie Keppard had a chance to make some recordings several years early, but passed out of fear people would imitate his style) - and not without reason! People used to cover their fingers with a handkerchief while playing so that their fingerings couldn't be seen, etc.

So it became common to rip the titles off of the charts so that onlookers couldn't see the name of the song being played. When asked, they would sometimes make things up - this is why a lot of the old songs have multiple names, or the same name for multiple songs, and all kinds of similar craziness.
