I cannot be replicated PHYSICAL ASSESSMENT: Alert, awake, conscious and coherent oriented to person, place and time. Glasgow Coma Scale of 15: eyes open spontaneously, obeys command and with normal power on both upper and lower extremities. Responsive to painful and tactile stimuli. Vital signs stable and with in normal ranges. Afebrile, skin is warm and dry to touch; with HR of 80 beats/ min regular in rate and rhyhm, synchronous with the PR. No heart murmurs noted, no S2 and S3 gallops noted, no jugular vein distention. Normal sinus rhythm on EKG, strong palpable perepheral pulse noted. BP of 120/70 taken on the right arm. Respiration rate of 20 breaths/ min, spontaneous and non-labored, no symptoms of dyspnea nor orthopnea noted, with clear bronchovesicular sounds noted on both lung fields, with equal chest expansion and tactile fremitus noted upon palpation. O2 sat of 99% on room air. Soft non-distended abdomen, with normoactive bowel sounds noted on each abdominal quadrant. Tolerates regular diet and fluids, no emesis noted. No history of anorexia. Able to micturate well, no bladder distention noted. No neurologic deficits noted, Cranial nerve functions are intact. With good visual acuity, peripheral vision intact. No myalgia noted, able to perform activities of daily living independently.
